Care Coordinator
The Care Coordinator (CC) works closely with the physicians, physician assistants, advanced practice registered nurses and registered nurses in providing care to the patients. The CC is responsible for scheduling diagnostic testing such as imaging, making referrals to other physicians, scheduling patient appointments, fielding phone calls from patients, completing disability and FMLA paperwork. Excellent interpersonal communication skills and an ability to organize and work in a fast paced environment is a must. The candidate will also be documenting in an electronic medical record. One to two years’ experience in the healthcare field is preferred but not required. Bilingual in Spanish and English is also preferred. High school graduates or GED required.
