Health Promotion Coordinator

This position is administratively responsible to the Direct Services Administrator for providing class instruction to older adults in a community setting and implementation of the mission, goals and objectives of the Agency on Aging \ Area 4. The Health Promotion Coordinator must be self-directed and have the ability to work independently, possess strong team building skills, demonstrate discretion and independent judgment.

Knowledge of:

  • Communication and organization.
  • Working in a team environment.
  • Customer service principles.
  • Basic and modern office procedures.
  • General understanding of the normal aging process and of the common needs/concerns of older adults age 60-100 and their family caregivers.
  • Familiarity with Long-Term Services and supports (LTSS), including community services, in-home services, and residential services.

Skills and Abilities:

  • Experience providing community education on healthy living, disease prevention, nutrition and exercise; health promotion and/or disease prevention programs that serve older adults in general, preferably on diabetes and/or falls prevention.
  • Knowledge of evidence-based programming.
  • Computer skills to maintain a computerized database, work with web-based systems use of Microsoft Office 2010 or higher, Windows 7 or higher, internet, e-mail.
  • Excellent verbal and written communication and strong interpersonal skills.
  • Analytical, critical thinking and decision making skills.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Attentive to detail and accuracy.
  • Ability to relate to diverse cultures, people with various disabilities, older adults and their families.
  • Ability to successfully adapt to a rapidly changing work environment.
  • Ability to gather, categorize, process, and analyze data efficiently and accurately.

Education, Experience, License/Certification:

  • Graduation from an accredited 4-year college or university with major course work in health education or health promotion, or a related field. Certified Health Education Specialist (CHERS) or similar specialized education is desirable but not required.

OR

  • Two-year degree from an accredited college, university or technical school,

AND

  • At least 2 years administrative experience with public programs that serve adults in general.

                                                                                                                                                                

 

Person-Centered Counselor

This position is administratively responsible to the Program Manager/Developer. The Peron-Centered Counselor must be a team player, be able to work in small groups and independently, one-on-one with consumers who may be in crisis.

Knowledge of:

  • General understanding of the normal aging process and the common needs/concerns of older adults age 60-100 and their family caregivers.
  • Personal or lived experience providing peer to peer or Person-Centered Counseling with public programs that serve older adults in general.
  • Basic knowledge of the Older Americans Act, Social Security, Medicare, Medi-Cal, Home and Community Based Services (HCBS), and other related governmental and private agencies providing consumer LTSS services.
  • Experience working with a diverse population including people with various disabilities, older adults, families, caregivers, persons in crisis, and LTSS providers.

Skills and Abilities:

  • Computer skills to maintain a computerized database, work with web-based systems, and use Microsoft Office products such as Word, Excel, Access, Outlook and PowerPoint.
  • Excellent verbal and written communication and strong interpersonal skills.
  • Strong analytical, critical thinking and decision making skills.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Self-directed and able to manage workload with little supervision.
  • Attentive to detail and accuracy.

Education, Experience, License/Certification:

  • Graduation from an accredited 4-year college or university with major course work in gerontology, social work, social planning or a related field,

OR

  • Two-year education from an accredited college, university or technical school,

AND

  • At least 2 years administrative experience with public programs that serve adults in general.
  • Bilingual and bi-literate preferred though not required.
