Waiver Services Provider - Approx. 20 hours/week in Elmira, NY
Waiver Service Providers (WSPs) are an essential component of the Bridges to Health (B2H) Waiver Program. WSPs provide B2H Waiver services in the child’s home, while others are provided at community-based sites.
To facilitate service provision, informed WSP representatives must attend team meetings for each child being served as follows: the first team meeting (held within 30 days of enrollment) and at least one team meeting every 6 months thereafter. WSPs must participate in the appropriate sharing of information related to delivery of B2H Waiver services with the HCIAs, the WSP agencies and LDSS:

3. Day Habilitation
4. Special Needs Community Advocacy and Support
5. Prevocational Services
6. Supported Employment
7. Planned Respite
8. Crisis Avoidance, Management and Training
9. Immediate Crisis Response Services
10. Intensive In-home Supports and Services
11. Crisis Respite
Bachelors Degree from an accredited college or university is preferred with two (2) years experience.
Must demonstrate the ability to work effectively with clients, families, staff and community contacts with a diversity of backgrounds and life situations. Capacity to participate in the principles and practices of the inter-disciplinary team approach. Good verbal and written communication, problem solving and organizational skills required. Must be creative and able to perform duties under general supervision. Excellent time management skills and ability to manage more than one project required. Working knowledge of computers required. Valid Driver's license required and must meet insurance liability requirements throughout employment. Lifting, moving and carrying of equipment/supplies may be required. Must be prepared and able to perform a variety of functions/activities with children on a daily basis.
