POSITION:
Community Based Treatment and Support Case Worker
DESCRIPTION:
There are immediate openings for Full Time, Part time and per diem caseworkers in our Community Based Services Treatment and Support Services program. Qualified and dedicated staff provide a range of behavioral health support services to youth with mental health and developmental issues in order to improve their health and well-being, and to maintain them in their homes and communities. These support services are also aimed to help the youth’s family manage the daily challenges of parenting a special needs child. Services are delivered in the youth’s home, school or other community setting, and are always goal-directed based on the individual needs of that youth and family.
Positions are available in Albany, Columbia, Dutchess, Rensselaer, Saratoga, Ulster counties, and surrounding areas.
Responsibilities include, but are not limited to:
Engage directly with youth and families to build a therapeutic, helping relationship.
Provide 1-1 teaching, coaching and rehabilitative counseling to build the skills and capacities of a youth struggling with mental health and developmental challenges.
Assist families to better understand and parent a child with mental health and developmental needs.
Advocate for a youth’s needs with other professionals including case managers, therapists, and school personnel.
Help identify non-health care needs (ex. housing, employment, social support) that may be impacting the youth and family, and connect them to community resources.
Timely and accurately document the delivery of services in an electronic health record.
Local travel to provide the services in the youth home, school or other community setting.
Maintain a positive, strength-based approach at all times.
POSITION REQUIREMENTS:
Bachelor's Degree required with 2 years’ experience, Master's Degree preferred.
Experience working with children with mental health or developmental needs, preferred.
(A High School Diploma or Associates Degree would be considered only with 3 years of direct experience working with children with mental health or developmental needs.)
Clean and valid NYS driver’s license.
Must have reliable vehicle for transporting clients - with proof of insurance.
Ability to work with developmentally disabled and/or mental health clients.
Ability to work flexible hours to accommodate client schedules which may include some early evening hours.
Ability to relate to a culturally diverse population.
Strong interpersonal skills.
Strong computer skills.
