Wooster Community Hospital Registered Nurse Case Manager
The Registered Nurse Case Manager will coordinate the care and services of selected patient populations across the continuum of illness; promote effective utilization of services; monitor health care resources; assume a leadership role with the interdisciplinary team to achieve optimal outcomes. Timely and appropriate discharges will be accomplished through assessment of the patients post discharge needs and the provision of services necessary to meet those needs. The case manager must be knowledgeable of community resources and alternate care facilities. This position must be able to handle multiple priorities and be self-directed. It requires good organizational skills and analytical thinking, and must be able to communicate effectively with physicians, staff, patient / family and outside agencies.
Requirements: Registered Nurse graduate of National League for Nursing Accrediting Commission or Commission on Collegiate Nursing Education School of nursing, possessing an Ohio license. Bachelor of Science in Nursing required. 2 years of case management experience in an acute care setting; or 2 years of nursing management in an the acute care setting preferred.
